
The most recent cosmetic surgery procedures statistics published by the American Society of Plastic Surgeons showed that the number of men undergoing a cosmetic procedure is actually on the rise. Despite that, there are definitely less men pursuing eyelid surgery compared to women, and this could be explained by several reasons, but the ones I hear frequently are fear of down time and changing the eye appearance.
Men who pursue eyelid surgery are two types, those who wait until the upper lid skin is so saggy that it droops over their eyes and as a result obstructs their vision. In this situation the surgery, which is called upper blepharoplasty, is deemed medically necessary to clear one’s vision and is covered by insurance. The other group is men presenting with aging changes around the eye who want to feel and look more youthful. The most common complaint is upper lid hooding or excess skin in the upper lid. This is not surprising given that men in general have eye brows that are positioned lower than women, which puts them at risk for sagging upper lid skin. Another common concern is deep lines and bags in the lower lid.
When doing a male eyelid surgery it is very important to maintain the masculine features of the male’s eye and face. For example both the upper lid crease and the brow travel in a gentle curve that has no distinct peak. Exaggerating these curves leads to feminization, which is hard to reverse. Also volume enhancement in the brow and cheek should be avoided or done very carefully for the same reason. As a general rule, when looking at a post operative photograph you should not be able to tell that a patient had cosmetic surgery until you see how he looked like before surgery and this is the essence of a natural look. For example when you look at this gentleman’s photo after surgery he looks like a normal guy and nothing really jumps at you, and that’s the goal.
